Chemical Pregnancy
The earliest loss, the worst-named diagnosis, and the honest numbers about what comes next.
A chemical (biochemical) pregnancy is a pregnancy confirmed only by hCG — a positive test — that ends before anything can be seen on ultrasound, usually within a week or two of the missed period. The name is terrible. There was nothing “chemical” about it: an embryo implanted, signaled its presence, and stopped developing. It is the earliest form of miscarriage, and it is a real loss.
The landmark study that defined this territory followed 221 healthy women with daily ultrasensitive hCG testing (Wilcox 1988). Of 198 pregnancies detected, 22% ended before the missed period — losses that, without daily testing, would have passed as an ordinary or slightly late period. Total loss after implantation was 31%. In other words: chemical pregnancies have always happened at this rate; ultra-early home testing simply made them visible. Almost always: nothing bad. Most chemical pregnancies result from chromosomal errors in that single embryo — a one-time event, not a defect in you. In the same cohort, 95% of women with an early loss went on to a clinical pregnancy within two years (Wilcox 1988). An implantation occurred — some fertility specialists even read a chemical pregnancy as modestly encouraging evidence that conception and implantation are working. Recurrent chemical pregnancies (several in a row) are the exception that merits a workup.
