What informed consent is supposed to mean
A signature is not consent.
Informed consent is a conversation, not a form. Professional guidance describes it as a process of shared decision making: you should be told what is being proposed, why, what the alternatives are including doing nothing, what the likely benefits and risks are, and how uncertain the evidence is. You should have the chance to ask questions and to say no.
The practical failure is timing. Consent for a cesarean is often taken in labor, in pain, sometimes after many hours without sleep. That is the worst possible moment to hear about placenta accreta risk in a future pregnancy for the first time.
The point of this toolHave the conversation now, while nothing is happening. Then in labor, the form really is only a form, because you already know what it says.
